As far as we could understand, the receiver is plugged in at the top of your GBA (SP). What we don't understand is where the microphone could possible be. Looking at the sketch, we don't see anything resembling a mic. However, this device was also rumoured to be a radio-receiver by Nintendo themselves, which makes more sense according to me...

The device was registered by Nintendo Japan on October 7, 2002. There's no information whether it is still in development, or cancelled. src: ipdl
